Summary:
Assesses plans, implements, documents, coordinates, monitors, evaluates, and updates the plan of care by collaborating with all members of the health care team to provide evidenced based care.
Establishes patient care management plans, interventions, treatment goals, and self-management goals.
Assists patients with self-management through education, visits and telephonic engagement, encourages and supports patient adherence to their care plans.
Researches, evaluates and recommends resources to meet medical and non-medical needs of patients and families.
Key Position Details:
2-3 days per week (days vary M-F).
More hours occasionally available on an as needed basis.
Job Description:
Principle Responsibilities Collects, prioritizes and synthesizes comprehensive data pertinent to the patient's health or situation.
Collects and prioritizes data in a systematic ongoing process that involves the patient, family, other health care providers and environment.
Evaluates, interprets, and coordinates information required to make decisions and recommendations to other heath care professionals regarding patient care and referrals.
Develops a plan that prescribes interventions to attain expected outcomes.
Develops an individualized plan considering patient characteristics or the situation in conjunction with the patient, family, community and others.
Establishes a plan that provides for continuity of care and identifies barriers and works to resolve them.
Incorporates evidence-based nursing practice and takes into consideration current statutes, rules and regulations when developing the plan of care.
Coordinates, implements and documents plan of care and outcomes.
Documents plan of care in electronic medical record.
Evaluates the patient's process toward attainment of the outcome.
Utilizes systematic and ongoing assessment data to evaluate outcomes and the plan of care.
Services on quality teams to improve quality of care and outcomes.
Collaborates with patient, family and others to effect change and generate positive outcomes through knowledge of the patient situation.
May schedule appointments for services.
Serves as a resource for patients, providers and staff regarding specific health plan and questions.
Other duties as assigned.
Job Requirements Associate's or Vocational degree in nursing required or Bachelor's degree in nursing preferred 0 to 2 years of experience working with people with disabilities or other chronic health conditions across continuum of care required Licensed Registered Nurse - MN Board of Nursing required if working in the state of MN Licensed Registered Nurse - WI Dept of Safety & Professional Services required if working in the state of WI BLS Tier 1 - Basic Life Support - Multisource required within 30 Days or ACLS/BLS Tier 2-Advanced Cardiac Life Support - Multisource required required for RNs who conduct cardiac stress tests and monitor sedation Physical Demands Light Work:
Lifting weight Up to 20 lbs.
